We will no longer tolerate the mistreatment, disrespect and violation of our familly members medical rights. This facility is a complete rat hole. She has been left to sit in her wheel chair from 1pm to 7pm after constantly pressing the button for assistance to return to her bed. Shes left slumped down in her bed literally all day because their too busy to shift her. She needs to be shifted from her bed to the chair way more often. She receives zero therapy, physical nor motor skills. Which is highly needed for her to improve. The aide is unable to understand what she is saying and she speaks clearly. She has been denied assistance in help getting dressed. Her purchased food is constantly taken from the fridge and eaten. When requesting to be changed to dry depends undergarment she is told shes not that wet to be changed. She has waited literally hours for assistance after using the call button. Its always were too busy for that right now or do you think ure the only patient that we have. Her food that she purchased was taken from her tray then thrown in the the garbage 13 minutes after it was delivered. The staff is extremely unprofessional including being rude. And the list goes on.. I pray that we can get her out of this facility as soon as possible. It has been a struggle speaking with the proper party regarding moving her. Again this neglect will not be tolerated.

My mother has been to this place twice for rehab services. They do not respond to the call bells for 20 to 30 minutes, sometimes longer. Ive waited on the phone with her while at work. When all other beds are filled at other facilities they are the only ones available. I dont wonder why!

My 96 year old mother has lived there for 4 years and loves it. The caretakers are friendly and really seem to care for her comfort. Before Covid, she got to go out to eat and go to Target to shop. They have lots of activities inside also. I would recommend it. I also stayed there for 2 weeks in rehab of a broken ankle. I felt pampered and if it werent for the pain of the broken ankle I would have enjoyed the it more.

I am not a fan of this place. Toxic environment. Difficult to get the support you need. Overworked. I miss the residents I cared for, but not the facility.

If I could rate this place 0/5 I would. Coming from a cna at another facility, DO NOT send your loved ones here. They are not capable of providing proper care, will leave a resident in their room for hours on end until something bad happens, and even then they wont notice for hours. They do not care about their residents and I will never be sending any of my loved ones to this facility.

I am the POA for a gentleman who stayed at Maplewood Care Center for over 7 months! The staff really went out of their way for him...from the laundry department, to the PT & OT with his prosthetic, to Ben in social work, to Diana in the business office, as well as all the TLC from those who cared or helped him with a matter. I know your jobs are not easy. Thank you so much!

I am really surprised to see all of the negative comments on here. My dad was there 6 years ago and our experience was great! Maybe because I was literally there to see him everyday and I did his laundry so there may not have been the opportunity for a bad experience. I don’t know. They had 2 great nurses that my dad loved. Roxy at the front desk was awesome and the social workers were great! My dad was there for about 3months. It saddens me to hear others have not had the same experience. We were on the first floor.

My sister is there for rehabilitation. HONESTLY..... her room is FILTHY!!!!! I took photos to prove it. There was at least one years worth of dirt behind the toilet, in every corner, cob webs, hair, spilled substances, peeling and chipping paint. Every nurse was from a foreign country, and I am pretty sure they may not even be competent. Yes, they are nice enough, but nice does not mean competent. Front desk staff are unfriendly. Asking to have the room cleaned by an RN.... even had her look at the filth... was not done 4 hours later. Someone should go in and do an impromptu investigation on them.

Of all the healthcare facilities Ive worked at (aside from the ones in Chicago), Maplewood Senior Living was a beautiful representation of diversity. Some days were exhausting, but most days were exemplary. I can honestly say I loved my team. The work ethic and determination to get stuff done; Simply amazing. May you all continue to grow and prosper in the spirit of VOAs founding Mission.

I wish that I couldve given my review without even giving this place one star. I wouldnt send an alley dog to this place. I had a family member that stayed here, and I got her out of that place as soon as I could. Theres a nurse that sits at the front desk that is rude, nasty, and down right unprofessional. Even the way that she talks to the aides is ridiculous. IMO she comes off as being racist because the minority people she spoke to really rude, but with other people she speaks to nicely. The room was nasty, and I waited there with my family member for over four hours to see how long it was going to take someone to even come see what I was talking about, but no one showed up, and the issue still hadnt been resolved the next day when I returned. Her phone came up missing, and when I reported it all of a sudden it was found at the front desk. The nurse came in to check my family members blood sugar (without gloves on), and then it took her over an hour to come back with her insulin AFTER she told her not to eat until she gave it to her. It took another 20 minutes for her to get her food warmed up, and that was only because I took it to the microwave, and when I walked down the hall there were staff members standing and sitting around, doing nothing, despite call lights being on (including my family members). This place really needs to be shutdown!

This place is the worst! Money came up missing, call lights on for hours at at a time. I called for two hours already and no one is answering the phone! I hate that my loved one has to be in this place. I feel so helpless, I can imagine how he feels!!!!

I am disgusted with this facility to say the least but will not get into details until my grandfather has passed with fear of retaliation. BUT, trust you me, I will make sure that no other human being will have to be treated the way he has been the past couple of weeks. I can say that it is almost impossible to communicate with the majority of the staff due to them not speaking English and them not answering or returning phone calls. This facility should NOT be in business and will be held responsible for their actions but only after we focus on my Grandpa being safe and cared for his last few days/ hours on this earth.

My Mother has been a patient there for over 4+ years. It is a marginal facility at best. Today I tried calling for over 5 minutes could not get a answer on there main line, so what does that say? Lack of staffing, lack of care? I think like all care facilities you need to be very careful where you end up. This would not be one I would choose for my Loved one.

My 61yo mom was there following a heart surgery and said its the worst place shes ever been (and has had plenty of surgeries). They stopped updating the place in the 80s and staff it with kleptomaniacs; they stole her coffee, her cocoa and her new package of underwear. She thinks they screwed up her meds, too, but were not sure if thats where that info fell through the cracks. Ultimately landed her back in the hospital. They charged for certain services that werent really delivered because equipment was broken and didnt work right. She did NOT mention anything about rudeness or abuse, only some inappropriate dramas, but still wouldnt recommend this facility.

This place is a JOKE, I have tried calling several times to speak with someone about billing. My mom was placed here after a stroke. Nothing like getting a call from a crying parent saying get me out of here. After arriving I had to walk through a smoke area in front of the main doors. What happened to 25ft from a health facility. Her room was filthy, the food trays(carts) have food all over the legs, you just feel gross after visiting. I monitored a green bean on the floor for the week my mom was in. Now on to her PT 20 min a day, 2 maybe 3 days per week.. But I bet you charged for a lot more. This place was nothing but shady. The social worker likes to play games when people are trying to transfer out so they can bill for more lack of PT.

If I could give them less than one star I would. If you have loved ones do NOT send them here. I find it suspicious that numerous people have given a 5 star rating to this facility with no comments. Maybe meant to boost their horrible rating. The care that would be received at this facility would not be worthy of an animal. There is no security people are free to roam, enter, and exit at will. We cleaned out my mothers room no one to be found. We could have stolen everything she owned. The place is filthy. Flys in the rooms. Shower day changes with the wind. They lie constantly. Bathroom call lights are a time roulette could be 10 minutes to an hour. They yell at patients. Drs not sure where their degrees are from, but orders may be getting mixed up. My mom received more injuries at this facility than care. OT and PT care was good and a very small handful of staff care NOT MANY though. Aides and nurses refusing requests to add barrier cream to pressure wounds. Some staff appear to have little to no training in normal care or just outright lazy. They are very nice when family are present, but decline in care when we were not. We took advantage of my moms hospital bounce back, due to doctor prescription error, to find a better facility for her. DO NOT SEND YOUR LOVED ONES HERE.

Dont work here unless you want to feel stressed and angry every day, especially with the staffing challenges. Not a lot of appreciation for work done from leaders. The reach for the stars stuff is not enough to show they care about you.

This is a fine facility. The people who work there are wonderful. Nurses are skilled and Nursing Assts are willing and able to help with any need a person has. My room was cleaned every day.The food was tremendous and home cooked. Therapy team worked well together. I was helped very much and they really cheered for my victories.And I always got my medicine on time.SincerelyMaryFrances Weightman

The nurse there dont know how to shower their patience have to wait until I file a report! When ringing the buzzer for nurse assistant no one comes so i have to walk to the front and tell them that her light has been on for 15 minutes plus. Smh

I stayed there for about three weeks and met so many freinds. The food was good top shelf compared to north west abbot hospital where the food was uneddable. The staff was great ben the social worker was amazing at taking care of any problems I had at a fast pace. The nurses were very helpfull expecialy aunna denaen and others. The therapists did excellent work hats off to jennifer and jennifer,grelda and so many that I forgot their names. They did amazing work that rewarded many patients. I would strongly reccomend this place the residents have such good hearts its a pleasure to know so many good people exist in this world. Signed Colin Youngberg

I stand with you ElizabethMy experience at Maplewood Care Center has been one of looking for meals. Im resident here. Breakfast so often is not available from Trina unless you pay her a personal visit. When I arrive after shes left. Other peoples names are left behind labeled covered breakfast plates with food under them are sitting at the closest table to her area. All breakfast food has been thrown away in the waste carts. To resort cold milk cereal( its good and healthy too). To find others their with not one made for me though others get their dinner meal sitting on the cafeterias dinner table. Many time over and over again.NAs, the nurse assistants are rotten; see you and dont know you! Not available to help out I caught two sleeping. Nate sits in their private restroom sleeps or uses his phone for long time periods. Whats he doing in their.I do not enjoy Nate since he let the person next to me in the room slip out of his hand white transferring him into a wheel chair without a body strap. An Mr. Donny lost his holding body air and down he went to the floor out of this chair hitting the floor with his head making a Bop thud sound. I heard this and thought what is going on over their behind the room curtain... Nate leaves Mr. Donny to exit the room.I get up to see ... Mr. Donny is on the floor on his back head down. No Nate. Back to my side of the room. In comes Chelsea a nurse; she says to Mr. Donny, what did you do to your self....?... Some weeks later Chelsea quits Maplewood Care Center.This place is ridiculous. To write the least....

Where do I begin? This place doesnt even deserve one star. I.M.O. it should be shut down. I cant imagine how anyone would allow their dog to stay there nonetheless a human being! My mother-in-law stayed there a few years ago and I never got around to writing a review, but this place still disturbs me so much that I have to write one as this place screams, Just roll over and die. Lets begin by there being no privacy. My M.I.L. had to share a room. Personally, I think having to share a room in a hospital, or anywhere, with a complete stranger is so uncomfortably difficult to do; not to mention privacy issues. To make it worse, the patients beds are only about 1 1/2 feet apart which are separated by a thin curtain in between - talk about seriously awkward! The rooms were soooo small, dirty, stale, and stinky it was nauseating! There was dirt and grime all over the floor. The toilet had wet and dried urine all over it - much more than just one or two days worth of dribbling on the toilet. The bathroom sink had a dirt-like grime in it as well. Next comes a serious depression risk from the lack of lighting: The rooms are very dark with very little lighting. If a patient is unlucky enough to get the side of the room with no window as my M.I.L., the room is very dark as s/he will get no natural light and there are hardly any room lighting fixtures either. One time, my husband and I had enough of visiting my M.I.L. in the dark, so I crossed the room into the other patients area to open the shade in attempt to get some sort of light in the room. Within 10 minutes, a rainstorm started and a nurse walked in and pulled the shade shut. I told her that the shade needed to be left open because the room was so depressing and some natural light needed to be let in. She responded that during storms, the shades need to be shut so that patients dont get upset by them. Are you kidding me? With everything so disturbingly horrible about this place, they are worried about a rainstorm upsetting the patients? There wasnt even a nice paint color on the walls or any pretty wall art to make the rooms a little more tolerable, (although it was so dark in the room, I couldnt tell exactly what color the walls were). Furthermore, I dont think that they bother bathing the patients very often. I could smell b.o. right when I walked in the room and I was surprised to see how oily and grimy my M.I.L looked - this is also true of the patient who was sharing a room with her. My husband and I werent even in the room for 10 minutes and we both started feeling depressed and agitated. If I ever need senior care and I have a choice between dying or having to live at the Maplewood Care Center to get the help I need, as of right now, I would choose dying. I had my M.I.L. transferred to Langton in Roseville. This place is beautiful with very good care. Even if rooms are shared, the rooms are very large and split in half by a large wall so it seems like patients have their own room - except for the bathroom. When Maplewood Care Center was informed that my M.I.L was going to be transferred, one of the staff asked why. Seriously!!!!??? Did this person loose her senses of sight and smell?

Poorly run. Dirty. Patients who are supposed to get a shower once a day (for open heart surgery wounds) are lucky to get one once a week. Gnats and flies everywhere. Most long term residents are dirty and should be in a mental institution instead of here. Do not send your loved one here!

Sadly disappointing. Resident is legally blind and not mobile in his chair on oxygen. We requested he be brought to activities, response was he has a list of activities in his room and can get there. Really?Resident dentures dont work asked for a soft diet, gets lettuce salad. Really?

Do not expect any healthcare at this facility. I found it to be archaic in medical treatment of any kind. No working wi-fi. No cares in regards to residents between set times: can wonder aimlessly, fall down, get stuck in wheelchair and left alone.If I needed meds ANY other time(s) than employees schedule - you wait from 45mins to ONE hour for a response - then wait for nurses to come - then wait until nurses get the meds. This was ANY meds, including insulin.I was served jello for dinner - came in disposable container with a fork - a fork that was missing a tine. The nurses are mostly rude and assume theyre doing you a favor - they do not care for you at all.This is no way to treat your family.

Terrible place. No one returns your calls. No one makes sure the patient is eating. If you tell them something is not right with the patient, they dont listen to you. They tell you they are running tests and they never do. Apparently they never have a doctor at the place. It is filthy. There are holes in the sheets. They never change the sheets. They lost my moms clothes and gave her someone elses mens clothes. No one can tell you what meds the patient has or had not taken and they lie to you and tell you its tylenol when its really morphine. They will leave a commode full of feces in the room all day until you complain but it was covered with a bathrobe. They will tell you that they are getting your mom out of bed but the giant infection on her back that they dont notice will tell you differently. Razor blades(open) were left on my moms tray for days and no one knew what they were for. DO NOT LET ANYONE GO HERE. My mom almost died from the lack of care for what was supposed to be a couple days of transitional care after surgery. She then had to be hospitalized and then back to another transitional care facility for a month to recover from this place.
