As advertised. The best entrance is from the West side of Pennsylvania Street, south of Ohio) Head west on Wabash Street, which is an alley between the parking garage and the Salesforce tower. Halfway down the alley on your right side youll see the door from the photos and the Amazon lockers are just inside. I was able to get in around 8pm on a weekday without a problem.

Its placed a little randomly, but I moved it to the right place on the map. Its accessible from the inside eating area (which closes at a certain time) and from an alleyway street that runs from Monument Circle to Pennsylvania Street. The latter entrance is open 24 hours.

You can go in through the alley way off Pennsylvania, next to the parking garage. Walk into the alley way and through a double door about 200 ft in. The locker is next to the elevators.
